If your child will be away from school for 5 days or more due to a holiday or family trip, this form needs to be completed and submitted to the school office for approval.
If your child requires any medication at school it needs to be prescribed by a doctor. We also need a parents authorisation to administer this medication. Please complete this form and give it to the front office with your child's medication if it needs to be given while the child is at school.
